Empire United Players and Families,

As we conclude the 2009/2010 Season, we would like for you all to take an opportunity to complete the End of Season Survey. Please take a few moments to complete the survey for each player in your family.  Your feedback is very important to us and will assist us as we enter into the 2010/2011 Season.

The survey is password protected and is available at the following location:

The password was sent out via email to all club families.  If you didn’t receive the email with the password, please contact your team manager.

The survey is broken down into 5 Categories:

• Player Specific
• Team Specific
• Organization Specific
• Contact Info (Only required if you want us to contact you directly)

Since this survey is for the entire Academy, please be sure to identify the location where you or your player participated.
